Medical Device Complaint Intake: Linking Technical Support to Safety Review

Customers may report safety information as a repair question or return request. Preserve the first report and connect complaint classification, potential harm, investigation and reporting assessment.

Key point — Start from what was reported, not the customer's label. A technical fix can resolve a service request while leaving safety assessment or reporting decisions open.

Distinguish inquiries, complaints and safety events

A question about use may require a simple explanation, or it may indicate repeated misuse caused by unclear instructions. An intermittent power-loss report requires consideration of product performance and patient impact. A sales call or messaging channel should not place the information outside the quality system.

KGMP Annex 2 section 8.2.2 addresses receipt and recording, assessment of whether feedback is a complaint, investigation, regulatory reporting, product disposition and corrective action. Preserve the original information alongside the classification. Do not rewrite the customer's account to fit the eventual conclusion.

Mark unknown information explicitly

Area Information Intake caution
Event First receipt or awareness, occurrence, symptoms and conditions Do not replace the earliest date with a transfer date
Device Product, model, lot or serial number, version and environment Do not assume an identifier from a similar model
People and impact Patient or user effect, medical intervention and recurrence potential Unknown is not the same as no harm
Evidence and follow-up Images, logs, returns, contact and next inquiry Handle only necessary personal data securely

Collect facts needed for urgent decisions first rather than insisting on a completely filled form. Unknown, unconfirmed and not applicable are different states. Accept the event even if product identification is incomplete and continue follow-up. Link original emails, images and call records to their source and receipt date.

Assess risk before setting the investigation scope

First consider death or serious harm potential, consequences of recurrence and repeated events involving a model or lot. Staff who cannot assess the risk need a defined internal route to quality or safety personnel. Improving the completeness of the intake must not delay necessary initial action.

Not every complaint is reportable or requires recall. Conversely, uncertain causation or the absence of harm does not automatically rule reporting out. Apply the event-specific conditions in the adverse-event reporting guide. This triage record does not replace statutory reporting timelines.

Connect service resolution with quality investigation

A restart or replacement part may satisfy the customer without resolving the cause. Service staff should retain actions, before-and-after condition, recovered parts and logs, then provide them to quality. Send the manufacturer the symptoms, circumstances, identifiers and completed actions together.

Do not suspend all internal reporting assessment and follow-up while waiting for the manufacturer. If a complaint is not investigated, document the justified reason required by KGMP. An unreproduced symptom is an investigation result, not an automatic closure rule. Link any returned-device hold and disposition to the same case.

Check four outcomes before closure

As a practical control, confirm the investigation or justified reason not to investigate, action for the customer and product, regulatory reporting or recall decision, and assessment of recurrence prevention. For reportable cases, connect submission evidence and follow-up information plans. A refund or completed service visit does not satisfy all four areas.

Exchange relevant information with manufacturers or distributors when outside activities contributed to the issue. If corrective action has begun but effectiveness verification remains open, track it explicitly. Case closure and longer-term improvement closure do not have to share an artificial completion date.

Keep classifications useful for trend review

Separate product family, symptom, cause and severity where helpful. Distinguish the suspected intake cause from the investigated cause while preserving original facts. Comparing raw complaint counts across models with very different supply volumes can mislead; consider exposure and severity.

Official sources checked on 2026-09-28: KGMP Notice 2026-46, Annex 2 sections 8.2.1, 8.2.2, 8.2.3, 8.4 and 8.5; Medical Device Safety Information Management Notice 2026-67. Intake and closure examples are practical controls; assess statutory reporting separately for each event.

Frequently asked questions

Q. Must the customer explicitly call it a complaint?
No. Assess the substance. Repair questions, returns or installation support can contain information about device quality, performance or safety.
Q. Can sales close a case if no patient was harmed?
That fact alone is insufficient. Assess potential harm, the effect of recurrence and whether complaint investigation or reporting assessment is required.
Q. Must a decision not to investigate be recorded?
KGMP Annex 2 section 8.2.2 requires a justified reason for complaints that are not investigated. Failure to reproduce a symptom does not justify skipping every assessment.
Q. Should we record sales intake or quality-team receipt?
Retain both separately. Do not overwrite the earliest awareness information with the internal transfer date; use the chronology to assess the applicable reporting trigger.
